Grave Worm
Large monstrosity, unaligned
A bloated, segmented horror that feeds on the dead. Grave worms are scavengers of the underworld, their tentacles tipped with paralytic venom that lets them feed on living prey when carrion is scarce. They leave glistening, reeking tunnels in their wake.

Scavenger's Stench. Any creature that starts its turn within 10 feet of the grave worm must succeed on a DC 13 Constitution saving throw or be poisoned until the start of its next turn. On a successful saving throw, the creature is immune to this stench for 24 hours.
Actions
Rotting Bite. Melee Weapon Attack: +5 to hit, reach 5 ft., one target. Hit: 11 (2d8 + 3) piercing damage plus 7 (2d6) necrotic damage.
Paralytic Tentacles. The grave worm targets one creature within 10 feet of it. The target must succeed on a DC 13 Constitution saving throw or be paralyzed for 1 minute. The target can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.
